Cancer Patients Should Ask Doctors to Use Simple Terms (HealthDay)
WEDNESDAY, Sept. 28 (HealthDay News) -- Cancer patients are often faced with many difficult-to-understand treatment choices that can have serious side effects and even mean the difference between life and death.
That's why it's crucial that patients insist doctors use plain language in explaining the options, advised Angela Fagerlin, an associate professor of internal medicine at the University of Michigan Medical School and a researcher at the U-M Comprehensive Cancer Center.
"People are making life and death decisions that may affect their survival and they need to know what they're getting themselves into. Cancer treatments and tests can be serious. Patients need to know what kind of side effects they might experience as a result of the treatment they undergo," Fagerlin said in a university news release.
She and her colleagues outlined a number of tips to help patients get the information they need to make an informed decision. These tips, published in the Sept. 19 online edition of the Journal of the National Cancer Institute, include the following:
- If you don't understand something, ask doctors to explain it again using simpler terms. "Doctors don't know when patients don't understand them. They want patients to stop them and ask questions," Fagerlin said in the news release.
- Focus on the absolute risk. There is often more than one way of stating the same statistic. A doctor might say, "This drug will cut your risk in half." Another way of stating that same fact is: "The drug will lower your risk from 4 percent to 2 percent." A patient armed with that information knows that their absolute risk is rather small to begin with, and can make a better decision. "It's important that patients and doctors know how to communicate these numbers, and patients need to have the courage to ask their doctor to present it so they can understand," Fagerlin said.
- Ask about additional risk. You may be told the risk of a certain side effect occurring is 7 percent. But if you didn't take the drug, is there a chance you'd still experience that side effect? Patients should remember to ask what the additional, or incremental, risk of a treatment is so they can make better decisions.
- Take notes. The amount of information given to cancer patients can be overwhelming. Either write it down, or at the end of a discussion, ask your doctor for a written summary of the risks and benefits. That way you can go home, have time to digest the information and make your decision.
- Don't pay too much attention to averages. Research that estimates the risk of getting a certain disease is often based on large groups of people, or populations, such as "people over 65 years old." But your individual risk may be lower or higher than the average risk depending on your genes, overall health, other conditions you may have, smoking status, etc. Your personal risk is what matters most, not the average risk.
- In some cases, there may be many different treatment options, but only a few may be relevant. Patients should ask their doctor to discuss options most pertinent to their individual situation.

Shanghai subway trains collide injuring more than 200 (Reuters)
SHANGHAI (Reuters) ? Two subway trains collided in central Shanghai on Tuesday injuring more than 200 passengers, though none critically, prompting public anger just two months after a deadly crash between two high-speed trains.
The collision occurred in central Shanghai, near the Yu Yuan gardens, a well-known tourist attraction, after a signal failure meant staff had to direct trains by telephone, state media said.
"As far as we know, more than 260 injured people have been sent to hospital, and there are no deaths so far," an official at the Shanghai government's propaganda department said by telephone.
Ambulances rushed to the station and emergency personnel were seen carrying injured passengers out, Reuters witnesses said.
Shen Jun, 23, who was in the first coach of the train that collided at 2:51p.m. (2:51 a.m. EDT), said "blood was everywhere."
"Many people were hurt. Look at the blood on me, on my arm," pointing at patches of blood on his shirt.
The official Xinhua news agency said most of the injuries were bruises and bone fractures.
"Today is the darkest day in the history of the Shanghai Metro's operation," said a news report by Chinese internet company Sina, citing the subway operator's official microblog.
"No matter the ultimate cause and responsibility, (we feel) particularly guilty about the harm and losses borne by the public. We will put in our utmost ability to rescue the wounded, resume operations as soon as possible ... and cooperate with the relevant departments in the investigation."
"Even if our apologies pale in comparison to the actual injuries, we are deeply sorry."
However, the statement on Weibo was later removed. It was unclear why.
The accident follows a collision between two bullet trains in eastern China in July that killed 40 people, which triggered public fury at the government's perceived slow response and accusations of a cover-up, expressed via microblogging sites.
That disaster also raised question marks over technology promoted as a symbol of the nation's growing prowess.
FURY ON THE INTERNET
The Shanghai crash quickly became the most talked about topic on the wildly popular Twitter-like Sina Weibo service.
"Another accident -- what a joke. So much money has been spent, all they've built is crap," wrote "ggirl."
"It seems like we are going to have to revert to ancient times when we walked on foot and communicated with people by shouting. But at least we now have high-tech Weibo," added Zhao Yingying.
The respected Chinese magazine Caixin reported on its website that the signals used on the Shanghai subway were made by China Railway Signal & Communication Corp. -- the same company that was blamed for the faulty signals in the July crash.
Calls by Reuters to the company went unanswered on Tuesday evening.
The high-speed train crash in July reverberated into China's stock market.
Sources said in August a $5 billion listing plan by the operator of China's new Beijing-Shanghai bullet train, initially targeted for next year, will be further delayed following July's train crash.
China Railway Group, the country's largest railroad builder, said earlier this month it had dropped a plan to raise about 6.2 billion yuan via a share placement due to uncertainty over regulatory approval.

Senators pass bill to avert government shutdown
WASHINGTON?? The agreement ensured there would be no interruption in assistance to areas battered by disasters such as Hurricane Irene and last summer's tornados in Joplin, Mo., and also that the government would be able to run normally when the new budget year begins on Saturday.
The Senate approved the resolution after a day of behind-the-scenes talks and occasionally biting debate, spelling an end to the latest in a string of standoffs between Democrats and Republicans over deficits, spending and taxes. Those fights have rattled financial markets and coincided with polls showing congressional approval ratings at historically low levels
The breakthrough came hours after the Federal Emergency Management Agency indicated it had enough money for disaster relief efforts through Friday. That disclosure allowed lawmakers to jettison a $1 billion replenishment that had been included in the measure ? and to crack the gridlock it had caused.
The Democratic-controlled Senate approved the measure on a bipartisan vote of 79-12, sending it to the Republican-controlled House for a final sign-off.
There was no immediate comment from House GOP leaders, although their approval for the measure seemed a mere formality after the party's Senate leader agreed to it.
"This compromise should satisfy Republicans...and it should satisfy Democrats," said Senate Majority leader Harry Reid, who added that Budget Director Jacob Lew had informed him that FEMA did not need any additional funding to meet its needs for the final few days of the budget year.
"It's a win for everyone," declared Reid, who had spent much of the past few weeks accusing Republicans of choosing to heed the wishes of tea party adherents rather than the needs of their own constituents battered by acts of nature.
Senate Republican leader Mitch McConnell of Kentucky said it was a "reasonable way to keep the government operational."
But he got in a final jab at Democrats, noting that the disaster funds sought by the Obama administration and its allies in Congress were now known to be unneeded.
"In my view, this entire fire drill was completely unnecessary," he said.
But not even the dispute-resolving agreement prevented Democrats from proceeding to a politically charged vote earlier in the evening that was designed to force Republicans to decide whether immediate aid to disaster victims or deficit concerns held a higher priority.

And the rhetoric was far harsher during the day on the Senate floor, when Mary Landrieu, D-La., unleashed an unusually personal attack on House Majority Leader Eric Cantor, R-Va., saying the weeks-long controversy started when he said, "Before we can provide help we need to find offsets in the budget."
She called that "the Cantor doctrine" and said the controversy "could have been avoided if Cantor had just said, 'I'm sorry, but I made a mistake.' But instead of saying that, he doubled down," she said.
Laena Fallon, a spokeswoman for Cantor, responded that the Virginia Republican had "never said the things she alleged, he has only suggested that we ought to provide disaster aid dollars to those who need them in a responsible way ? something that she's voted to block despite the urgent need."
In fact, House Republicans insisted that any new disaster aid for the expiring budget year be offset by cuts elsewhere in the budget, a decision that Democrats seized on in hopes of reshaping the political terrain to their advantage.
Because the House is on a one-week break, it was not immediately clear how the legislation would be cleared for President Barack Obama's signature.
Among the options are passage of a temporary funding measure, to be passed in a brief session of the House planned for Thursday, that would keep government agencies in funds until lawmakers return on Oct. 4. The Senate approved the bill without objections.
Alternatively, GOP leaders could call the full House back into session this week for a vote.
Either way, the agreement assures funding until Nov. 18.
FEMA spokeswoman Rachel Racusen, said the agency had $114 million left in its disaster relief fund, enough to last until Thursday or Friday, the final business day of the current budget year. She said the exact timing would depend on the number of emergency victims who apply for aid, and whether any new disasters occur.
FEMA officials had said previously the funds would run out early this week. That concern prompted the Obama administration a few weeks ago to ask Congress to approve a replenishment to tide the agency over through the Sept. 30 end to the fiscal year.
House Republicans agreed weeks ago to provide $1 billion and include the money in a bill that also provides money for most federal agencies for the first few weeks of the 2012 budget year. At the same time, they insisted on cutting spending elsewhere in the budget by $1.5 billion to prevent the deficit from rising, an amount later raised to $1.6 billion.
That, in turn, produced a quick attack from Senate Democrats, who opposed cuts.
While it was unclear precisely how long FEMA's remaining funds would last, one official said the agency began conserving funds last month as Hurricane Irene approached the U.S. mainland, prioritizing its aid to help individual disaster victims and pay states and local governments for immediate needs such as removing debris and building sand bag barricades.
Funding of $450 million has been put on hold for longer-term needs such as reconstruction of damaged roads, the official said. In addition, the agency has been able to reclaim unused money from past disasters, according to the official, who spoke on condition of anonymity, citing lack of authority to discuss the matter publicly.

EasyJet founder wants to launch new, rival airline (AP)
LONDON ? The founder and biggest shareholder of easyJet is planning to launch a new, rival airline, the budget carrier said Monday.
EasyJet said it received notice from Stelios Haji-Ioannou that he plans to launch the new brand soon. There were no immediate details on exactly when the new carrier will launch, or how it will differentiate itself from budget carrier easyJet.
A website for the proposed new airline simply reads: "Fastjet.com by Stelios. Coming soon!"
Stelios, who founded easyJet in 1995, remains the owner of its brand. He and his family own 38 percent of the company's shares.
But the entrepreneur has been embroiled in a series of conflicts with the airline.
The disputes included a legal battle over the use of the "easy" brand license and Stelios' criticism of easyJet's expansion plans.
EasyJet said Monday it would act if Fastjet or Stelios' other companies infringe on rights previously agreed between them.
It said it would seek "constructive dialogue" with Stelios and his private company easyGroup.

British police: 6 charged in terror plot
LONDON (AP) ? Six men have been charged with involvement in a terror-bomb plot, police announced on Sunday.
The arrests last Monday of the six Birmingham-area men were part of a "major operation" by the West Midlands counterterrorism unit, the West Midlands police department said.
Police said Tuesday they had been given until midnight Sunday to charge the men, release them or apply for a further extension.
Four were charged with preparing for an act of terrorism, and two more with failing to disclose information, the police said. One of those two was also charged with terrorist fundraising.
All six, who are aged between 25 and 32, will appear at West London Magistrates Court on Monday.
The police said Irfan Nasser, 30, Irfan Khalid, 26, Ashik Ali, 26, and Rahin Ahmed, 25, were charged with plotting terrorist acts.
They were involved in planning between Dec. 25, 2010 and Sept. 19 of this year to prepare or help others prepare to commit acts of terrorism, the department said.
Nasser and Ali are alleged to have planned a bombing campaign; stated an intention to be a suicide bomber; collected money for terrorism; made or helped others make a bomb; and recruited others for terrorism, police said.
Nasser and Khalid are also accused of traveling to Pakistan for terrorism training including bomb-making, weapons and poison-making, as well as making a martyrdom film, police said.
Ahmed is accused of helping others travel to Pakistan for terrorism training, collecting money for terrorism and investing and managing terrorist funds, police said.
Ashik Ali's brother Bahader Ali, 28, and Mohammed Rizwan, 32, are both charged with failing to disclose information about acts of terrorism, police said. Bahader Ali is also charged with terrorist fundraising.
A seventh Birmingham man, aged 20, who was arrested Thursday, is still being questioned, officers said.

I?ve always believed that the best science is science that effectuates tangible and healing change. Healing happens on many different levels, from the global ecosystem known as Earth, to the microscopic compartment called the Cell.? Somewhere between Earth and Cell lies you and me ? human beings.? And how we interact with both each other and our environment has profound consequences on all types of health.? Global Health.? Personal Health.? Cellular Health.? And a particular domain that I am now very interested in ? Community Health.
Anyone who has read Chapter One of Malcolm Gladwell?s Outliers has at least considered the value of nurturing our social and family networks. In Chapter One we learn of a hypothesis that the social connectivity within a community promotes physical well being.? I suggest that the consequences of social well being reach far beyond one?s local community.? How we interact within our family and communities could very well influence the rate of global warming.
How do we mitigate global warming?? I suggest domestication.? Webster offers up two definitions of the word, ?domestication?:
1.? ?to bring to the level of ordinary people?
2.? ?to adapt (an animal or plant) to life in intimate association with and to the advantage of humans?
The first definition speaks to how, even as a PhD trained geneticist, I am quite ordinary with respect to the statistical and experimental technicalities of global climate change. So I trust the experts ? global warming is happening. But ordinary people like me need ordinary solutions. What can I do today to promote the health of Mother Earth?
Which brings me to definition two.? Let?s domesticate the mitigation of global climate change to every and all advantage.? Even to the advantage of my community and cellular health.
And so I posit several ordinary measures!? Enjoy.
Less is more.? More is less.? May you have enough.? Want policy change?? Kick the current leaders out; let more ?Less is More? Euro-loving politicians in to political office. Even if they?re philosophical hipsters. Two cars really are twice the hassle, expense, and gas; try one car per household. Beef up commuter rail. 4000 sq feet is actually a lot of home; restore or build small. Nursing homes for every Baby Boomer will not be possible; bring the grandparents back home. You?ll learn more living with your grandparents. Trust me. Just because it?s food doesn?t mean it?s good for you. Eat local. Eat well. Eat less. Eat with other people. Recycle everything you can. Go Strawless. Let?s talk about job creation; make it socially acceptable again for a father or mother to raise children without another job. Less gaming and less activities that involve LED, LCD, or plasma screens; more nature. Have two kids; consider the possibility that more than two children might contribute to overpopulation, which is actually the greatest threat to Life on Earth. Sorry folks we get one Earth, and I?m not sure we?ll evolve quickly enough to either land on or live on the Moon, Mars, or Venus. Mitigation through domestication is an incredible win-win for both future generations and the communities we take part in here today.
